- allow to set the info file name

- correctly generate code for nodes like:

I've attached my revised patch that adds support for some
additioanal markup constructs beyond the previous version of
the patch.  Taken relative to the CVS HEAD.

I'm sure there are things that need fixing, but it'll take
specific reports and patches to get that done -- I just
don't have the time available.
